親愛的應屆畢業生:

倘若已確定無法完成雙主修、輔系、學程之修讀而想放棄身份者，請於5月6日至5月8日提出申請！

申請流程

• 申請人請親自填寫放棄資格申請書送請輔系、雙主修或學程系主任審查→主系系主任審查→申請書交由註冊組彙辦。
• 申請人若已修習部分科目，應另檢附「在學中文歷年成績表」一份並填寫「放棄科目學分核定表」送請主系系主任審核，核定表交由註冊組彙辦。
• 本申請書送交註冊組完成手續後，不得以任何理由要求恢復雙主修、輔系及學程資格。
• 本次申請僅為放棄身份，已選課者仍須完成本學期已選修之課程。

Abandonment Applications for Double Major, Minor Study, and Credit Program (only for graduating students of the Academic Year 2023)

If graduating students of the Academic Year 2022 have enrolled in any double-major, minor-major or any credit program, please attend the class based on their course schedule till the end of this semester If they cannot complete their double major, minor major or any credit program before due day, they can submit applications to relinquish their double major, minor major and the credit program and then graduate from the department of their original major.

Submission guidelines:

All applications shall be need to hand in their applications to

1.      All applications shall be need to hand in their applications to (1) the Chairperson of the double major department, (2) the Chairperson of the original major department, and then (3) the Registration Section for the purpose of approval and processing between May 6^nd and May 8^th.

2.      The needed documents include (1) the application form to relinquish one’s status, (2) a transcript in Chinese and (3) the credit relinquish form of one’s minor major/double major/credit program.

3.      Once the procedure is completed, reinstatement of one’s minor major/double major/credit program will not be permitted.
